Phyllis Ann Green, age 75, passed away Friday, December 29, at the Lee County Care and Rehabilitation Center in Beattyville, KY.
Phyllis was born September 8, 1948, in Oneida, KY a daughter to the late Orlie Lee and Charlene (McIntosh) Green. She retired as an LPN for the Lee County Care and Rehabilitation Center in Beattyville, KY. She loved her family and enjoyed spending time with all of them.
She is survived by 1 son; Buddy (Tina) Cornwell, 2 daughters; Marilyn Kaye (Vada) Wilson and Nora Lynn (Jeff) Ragan, all of Booneville, KY, 1 brother Clifton (Kathy) Green of Campton, KY,1 beloved brother in law; David Collins of Richmond, KY,1 niece; Rhonda Collins and nephews; Randy and Jerry Baker, Josh (Jessica) Green of Campton, Ky,1 special niece, Jessie (Will) Jewell of Campton, KY, 6 grandchildren; Jeremy, Logan, Samantha, Makayla, Luke and Chloe, all of Booneville, KY, 9 great grandchildren; Adriana, Landon, Camille, Braun, Ryker, Raegan, Cayson, Mckenna and Evelyn. along with many other loving family members and friends.

Phyllis was preceded in death by her parents, and 2 sisters; Pam Collins and Gloria Baker.
A memorial service will be held on Saturday, January 6, 2024 from 2:00 PM to 4:00 PM at the Booneville Funeral Home.
